By signing this release, Waiver and Assumption of Risk, (the “Release”), I agree that Worship Arts Dance, LLC shall not be responsible for any injury suffered by the student (“student”). The Release extends to LaTonya Reagor, staff, guest instructors, volunteers and locations in which classes, workshops, and/or events are held. I acknowledge that I understand the nature of the activities my child/I will be participating in and that my child is/I am in the proper physical condition and capable of participating in the related activities, understanding that Worship Arts Dance, LLC or LaTonya Reagor is not in any way responsible for making such a determination. The Release extends to every claim, demand or liability of any kind based on any injury or damage described below. _____ (Initial) I understand that dance activities as conducted and taught by LaTonya Reagor and Worship Arts Dance, LLC have inherent risks of injury. These risks include, but not limited to, muscle pain and pulls, broken bones, ankle injuries and other personal injury. I recognize that the student is exposing himself/herself to such risks when undertaking dance activities. I understand that these risks cannot be fully eliminated without jeopardizing the essential qualities of the activity. The student and I assume and accept all risks of injury or damage resulting from such dance activities. The student’s participation in this activity is purely voluntary, and the student elects to participate, and I join in that election, in spite of the risks. ______ (Initial) I also agree that LaTonya Reagor and Worship Arts Dance, LLC is not responsible for any property loss or damage suffered by the student, or any guests or parents of the student, which results upon entry into or presence in the class or its surrounding property. I understand that LaTonya Reagor and Worship Arts Dance, LLC is not responsible for any student(s) under any adult supervision who are left unsupervised in the common areas and areas surrounding the class location and that LaTonya Reagor and Worship Arts Dance, LLC will only be supervising the student(s) when he or she is participating in scheduled dance activities, programs or instruction. _____ (Initial) Please see our COMPLETE "Release Form Liability and Waiver of Claims for Dance Instruction". All registrants will receive a full copy with additional liability and waiver clauses. Please request a complete "Release Form" in its entirety prior to registering.
